What is an EDI 274?

The EDI 274 Healthcare Provider Information is the ANSI X12 transaction set used to exchange demographic and educational or professional qualification information about healthcare providers between providers, provider networks and any other entity that maintains or verifies healthcare provider information.

Health plans maintain provider directories and credential providers before they join networks. Accurate data on locations, specialties, licenses and whether providers accept new patients matters to members and regulators. Exchanging it in a standard format reduces the burden of repeated data requests on providers.

How to read this diagram: the top row shows who sends the 274, to whom, and which documents usually come before and after it. The lower part shows what X12 release 008010 itself requires: the envelopes around the 274 and the segments and loops the standard marks as required, area by area. Optional loops are listed by the segment that starts them rather than drawn in full. This is the standard's minimum, not a trading partner's requirement: each partner's implementation guide usually makes some optional loops mandatory, restricts the codes and may use a different release.

Structure

Mandatory segments in the 274 (X12 release 008010)

The segments X12 release 008010 marks as mandatory at their level, in order. Segments inside optional loops are needed only when the loop is used. This is the standard's requirement, not your trading partner's: each partner's implementation guide names the release it uses, adds the optional segments it requires and restricts the values. The requirement status comes from one published X12 reference listing and has not been checked against a second source.

Practical example

The 274 in practice

A medical group adds two physicians and opens a new clinic. It sends its contracted health plans a 274 with the new providers' identifiers, specialties, licenses and the clinic's address and hours. The plans update their directories and start credentialing the new physicians.

Implementation

Implementation considerations for the 274

  • Use national provider identifiers where applicable.
  • Keep directory data current to meet regulatory requirements.
  • Validate licenses and credentials before activation.
  • Protect providers' personal information.
